NOTES: Originally I had this labelled as a "promo".  Upon reflection that word was used
incorrectly and caused some confusion.  The source for this dvd was not a promo, but archive
material used by the band/management.  The VHS tape used for this dvd was actually sold on
ebay some months ago and used the term "promo" - which in reality it is not.
